PARTICIPANT OR PARENT GUARDIAN AGREEMENT

Release of Liability.

I, and or the parent/guardian of the registrant, a minor, agree that I and the registrant will abide by the rules of Hotbed Lacrosse, LLC., dba Hotbed Lacrosse Camps/Clinics, Hotbed lacrosse Teams; ie HotStix (the "Clubs") Recognizing the possibility of physical injury associated with Lacrosse, I hereby release, discharge and or otherwise indemnify the Clubs Owner(s), Director(s), Coaches, Staff, Club Sponsors, their officers, staff, employees and all associated personnel against any claim by or on behalf of the registrant as a result of the registrant's participation in the Clubs. As the parent or legal guardian of the participant, I hereby give my consent for emergency medical care prescribed by a duly licensed Doctor of Medicine or Doctor of Dentistry or any other health care official. This care may be given under whatever conditions are necessary to preserve life, limb or well being of my dependent. I understand that the Clubs do not provide or supply insurance to its participants, that medical coverage is the responsibility of each participant.

Authorization to Use of Photographs and Images.

I do hereby grant permission to the Clubs. And its owner(s), officer(s), trustee(s), employee(s), agent(s), representative(s), successor(s), licensee(s) and assign(s) to photograph the image of the registered player herein, likeness or depiction. I hereby grant permission to the clubs to edit, crop, or retouch such photographs, and waive my rights to inspect the final photographs. I hereby consent to and permit photographs of the registered player herein and or those of my minor children to be used by the Clubs worldwide for any purpose, including educational and advertisement purposes, and in any medium, including print and electronic. I understand that the clubs may use such photographs with our without associating any names thereto. I further waive any claim for compensation of any kind for the clubs use or publication of photographs of me, the registered player or those of my minor children. I hereby fully and forever discharge and release the club from any claim for damages of any kind (including, but not limited to, invasion of privacy; defamation; false light or misappropriation of name, likeness, or image arising out of the use or publication of photographs of me, the registered player herein or those of my minor children by the clubs, and covenant and covenant and agree not to sue or otherwise initiate legal proceedings against the clubs for such use or publication on my own behalf or behalf of the registered players herein or on behalf of my minor children. All grants of permission and consent, and all covenants, agreements and understandings contained herein are irrevocable.

Refund Policy:

HotStix Team Refund Policy:

The Player Application Fees are non-refundable. Upon written request via US Postal Mail, registered participants will receive a refund of deposit(s) paid less a 5 percent processing fee of the deposit fees only prior to the close of online official registration. Registration dates are posted at the time of registration. Should a registered participant, selected to a team roster be unable to play due to physical injury sustained during a school activity, the clubs policy is to refund monies paid less a 5 percent processing fee only after receiving a written account of the injury from the registered participants physician. Registered players that have been selected and placed on a team roster will be entitled to receive a refund of 50 percent of the monies paid if the registered player does not participate for any other reason whatsoever. Check refunds will be processed by the clubs.

Hotbed Lacrosse Camps Refund Policy:

If, for any reason, you are unable to attend a camp for which you have registered, a credit will be issued for all moneys collected. This credit can be used by you or any family member for any HotBed Lacrosse Camp/Clinic/HotStix Team through the end of the following year the credit was issued. For example, a credit issued in May, 2008 is valid through the end of 2009.

If the camp is cancelled or amended in any way due to inclement weather conditions such as rain lightning or excessive heat, no refund will be provided for the part of the camp that is missed. We will attempt to schedule additional time in the same week if possible.

In the event a participant must leave the camp early and cannot return, a pro-rated credit will be issued that can be applied to the fee/fees for any Hotbed Lacrosse Camp/Clinic/HotStix team through the end of following year.

By registering my child for a lacrosse camp, clinic or team I am giving my consent and electronic signature.
